Failed BRS repack

My hanger buddy had his BRS 1050 repacked. We fly out of Meadowlake Airport at 6900'. On a flight over 12k the clear inner seal ruptured. He sent it back in for repair and the 2nd repack ruptured on his kitchen table. He lives at 8800'. BRS said they have never had this problem before. Has anyone else experienced this. The cables on his canister missed being pulled into his prop by 4 inches. They have now put in a gortex type of vent for this 3rd repack and so far no trouble. Yes we can fly at 12500 regularly as the sport pilot rules changed on April 2nd.Thanks, Brian
